ALL STUDENTS ARE REQUIRED TO SUBMIT A KINGSPORT CITY SCHOOLS MEDICAL FORM BEFORE PARTICIPATING WITH THE DOBYNS-BENNETT BAND.  

FORMS MAY BE TURNED IN TO A PARENT VOLUNTEER DURING BAND CAMP OR PLACED IN THE GREY MAILBOX IN THE LOBBY OF IMB.
